Registration
for the Fall 2012/Spring 2013 season is NOW OPEN and will close July 10, 2012
IMPORTANT:
Payment and a copy
of your child's Birth Certificate (for new players) are required at the time
of registration.
Children WILL NOT BE ELIGIBLE FOR PLACEMENT on to a team roster unless these two requirements have been met.
Birth Certificates can be mailed, faxed, or emailed to our office or you can deliver it in
person. Some returning players may also have outstanding birth certificates and you will be notified by the office
staff.

to "Play-Up"
If you are requesting your child to "play-Up" to an older age group due to school grade peer group, please print out
this form and when completed and signed by the school principal, send it to the T-CYSA for proper processing and
consideration.
